I have posted a page with my cellar book form on my website. I have been using this form for several years. I have Kinkos print them and punch holes in them for a regular three-ring binder. It has served me very well. You are welcome to use it if you choose. It is in Excel so you can customize it if you want. I keep this written record in the cellar along with an inventory list in Excel on a computer for insurance. I enjoy hand-writing all my purchases and tasting notes/events. I can really remember things much better that way.
